The Family Connection is a DMH funded wraparound services project
serving at risk children/adolescents (to the age of 19) and their
families who reside in one of the eleven communities of the upper
Merrimack Valley. Wraparound entails short term, strengths based,
flexible community services designed to stabilize the family and
reduce the risk, frequency and duration of a child's out of home
placement. Examples of wraparound services include family advocacy
and networking, after school activities, respite, mentoring, summer
programming and family activity opportunities. Referral packets are
completed on a child's behalf by the parent and a provider, selected
by the parent as the referral agent, who serves a case management
function. Services are coordinated and brokered within the family's
area of meaningful ties via a team meeting, consisting of
professional community providers, the child's parent(s), the referral
agent and other providers, and the wraparound director. Wraparound
services are recommended for 3-12 months with required review as
needed to assess benefit of continuation of any service.

We meet with eligible families from the eleven listed communities.
Each family has one family team meeting to coordinate existing
services and develop a service plan.
Each team meets twice monthly.
Teams are comprised of the family, their selected referral agent,
providers and members of their support network, The Family Connection
Director and professional representatives from DMH, DSS, MSPCC,
Parent Coordinator, School Departments, District Court Probation and
ad hoc monitors representing the local community.

Helping families identify their needs and strengths
Assisting families to access and enhance community
supports and resources
Supporting opportunities for families to network with
other communities
Educating families toward increased confidence in
parenting their challenging children
Providing specific creative and flexible services to
reduce out of home placements for children at risk
Empowering Families to become strong advocates for
their children
Improving service coordination and collaboration among
families, providers and child serving agencies

Families with children under 19 years of age who

Families who are parenting children with serious
emotional disturbances
Families parenting children whom are at risk of out of
home placement
Families who are involved with at least two
child-serving agencies, one of which may be school Special Education services
