The purpose of this service is to support the client's treatment and his/her tenure in the community.   Assistance with accessing resources in the community and collateral contacts to coordinate care with medical and mental health providers.   Emergency Services, Hospital/ART/CSU social workers, professional staff working with the client may contact the insurance company of the client by faxing completed forms to make a referral.
