The program will serve six medically ill and mentally ill clients who are challenged in their ability to manage medical illnesses and the symptoms of their mental illness in a community setting.   SPECIALTY SERVICES Staffing will include traditional mental health workers as well as licensed practical nurses. A part time nurse practitioner will work with clients to provide primary care if needed and work with the clients PCP’s in the community.   REFERRAL PROCESS Tewksbury State Hospital will refer clients.
