ASSERTIVE COMMUNITY TREATMENT (ACT)
ACT is a Department Of Mental Health (DMH) funded 24/7 wraparound program designed to provide support to clients diagnosed with severe and persistent mental illness who live independently in the community. Through the activities of a small, energetic, multi-disciplinary team clients receive highly individualized psychiatric and rehabilitative services in their home, community or at ACTs office depending on the environment determined to be most conducive to both the clients treatment needs and personal preferences.
Current initiatives include incorporation of health and wellness with psychiatric care through group and individual nursing support. Also the team has identified 5 slots specific for Transitional Age clients.
Referral to ACT is managed exclusively through the Department of Mental Health, Beverly Site Office. Interested consumers should be directed to contact their DMH case managers for further information. DMH eligibility is a requirement for admission.
